Available Form Formats Information
Welcome to the Florida Department of Transportation's Forms Page. This web page allows you to search and access information about the Department's forms. The page provides information such as form number, title and the last revision date.
The information you will view is accessed from a live database in the Forms and Procedures Office. Therefore, the information you are viewing is always current. As changes occur, the database is updated and the changes are immediately reflected on this page.
The forms on this site are the official forms of the Department and are managed and controlled in accordance with Procedure No. 050-030-001, Form Development and Control (PDF 137KB). Official forms of the Department cannot be revised or altered without authorization from the originating office. See Word Processing Forms below for additional information.
- HTML: As of March 2003, the Department
began migrating forms on the Internet to an HTML
format. The software is server based and does not
require any components to be installed on the user's
computer, and it's faster (takes less time to
download). The new product (called Adobe Form
Server) takes the Jetform that we are using today
and runs it on the server, and renders the form in
several device friendly formats such as HTML and PDF. : The Department is making every effort to migrate all forms to a more web compatible format, therefore this page has been designed to allow for multiple formats.
- PDF FORMS: Most forms will be available in PDF format, which requires use of the free Adobe
Reader software. If you do not have the Adobe Reader, you may download a free copy here
- WORD PROCESSING FORMATS: For convenience of our external customers, some forms may be available in word processing format, such as Word. In using the word processing file, the content of the word processing file on the web is the official version of the form and changes or additions to the form are prohibited. Word processing forms for legal documents such as permits that are incorporated by reference into a rule may have a disclaimer clause added at the end for the user to sign representing that no change has been made.
- WORD FORMS: Some of our forms are
listed as Word Forms. This are designed with
fillable fields for input, but have the rest of the
form locked to prevent changes. Some of the
normal features of Word are not available in this
format due to them being locked: Spell Check
is foremost among these. However, we have
added a macro version of spell check to the majority
of the Word forms. You will need to enable
macros to allow this to work. These documents
should be saved BLANK to your local computer.
- EXCEL: A few forms are available in Excel
format. These function similarly to our Word
forms. They may be downloaded and completed
offline.
- OTHER: If a form is not available in electronic format or if other instructions apply, you may obtain this information by clicking on OTHER by the form title.
