Forms & Procedures
The Forms and Procedures Office is the main repository for the Department’s
forms and has the sole responsibility for form authorization, creation, and
maintenance. This includes forms that are designed to integrate with approved
Department applications. Unless an exception is granted, all electronic versions
of forms are to be created, accessed, and controlled through the official Forms
and Procedures Office and approved by the Forms and Procedures Office.
It is the responsibility of the Forms and Procedures Office to determine if a form meets the definition of an official form and also to ensure the integrity, availability and access method for all Department forms. Likewise, the Forms and Procedures Office is responsible for determining the technology in which an electronic version of a form is to be created and utilized.
