Official forms of the Department of Transportation are managed by the Forms and Procedures Office. Additionally, we maintain Procedures (which include Directives, Standards and Guidelines), Policies and Manuals.

    The forms on this site are the official versions of Department forms. Since many Department forms have legal implications, and in accordance with FDOT Procedure No. 050-030-001, Form Development and Control, they cannot be revised or altered without authorization from the originating office. The originating office and contact person names are available by clicking on Detail for each form. Please note special instructions based on file format:

    Interactive PDF: These files require the use of Adobe Acrobat Reader. Interactive PDF files can be filled in electronically, saved, and printed. When saving an Interactive PDF, the file will save to the Forms server and can be retrieved by the creator at anytime. NOTE: Interactive PDFs cannot be saved to your computer when using Acrobat Reader. For more information, view the Interactive PDF tutorial.

    Read-Only PDF: These files can be viewed using Adobe Acrobat Reader and printed.
         Word Forms: These files can be filled out electronically, saved, and printed. Word files are locked, which means that you will only be able to edit field entries, not the form itself. You MUST save Word forms as a .doc extension for them to function. Word 2007 often defaults to .docx, so be sure to change this when saving a Word form.

    Other:
    The form is not available in electronic format or other instructions apply.
