Construction
In an effort to streamline the submission and acceptance of
required forms, the Department is allowing the potential
electronic submission of many forms. Please review the
guidelines and processes below on how to begin successfully
submitting forms electronically. Forms currently approved
for electronic submission are also listed below, separated into
2 sections based on whether the form requires a signature.

| The forms identified below may be submitted electronically to the appropriate FDOT staff members. | |||

The forms in Section 1 may be completed and emailed to the
appropriate personnel (e.g., Project Administrator). Since these forms do not require a signature, it is not necessary to print and/or scan these forms. |

The forms in Section 2 do require a signature for the form to be
considered complete. These forms may be completed and
emailed to the appropriate personnel (e.g., Project Administrator) using either of the methods
described below:
1. Print the completed form and obtain the appropriate signature(s). Scan the signed document and attach to an email sent to the Project Administrator.
2. Complete the form and use an approved electronic
signature to sign the form and email to the Project
Administrator. |
