The files below are all self contained downloads. Please choose the Save option when prompted for the best results when using the forms.
After choosing Save, you will be prompted to store the file in an area on your local hard drive or in a network storage server. Please choose a location that is easily accessible.
After a location is chosen and the file has been saved, just browse to the file and double-click it to run the self-extracting function. In the picture below, just click the .exe file, and the contents will be unzipped to an area of your choosing.
The screen shot below shows the file being placed on your local C:\ drive, but you can choose alternate areas by clicking the Browse button.
Finally, the actual file will be place in the area you specified and you can open it for viewing whenever necessary.
