District Construction has created this spreadsheet to assist the Project Administrator in their efforts of cataloging drainage system deficiencies identified during a review of the DVD's and Reports provided to the Department in accordance with Specification 430-4.8.   

For each deficiency discovered in the drainage system, the PA should compile the information identified in this spreadsheet, as applicable. The PA should then submit the spreadsheet along with the appropriate DVD, Report and the Contractors proposed repair method for each deficiency to Paul Harkins of the District One Construction Office.

District Construction Office review and concurrence is required prior to authorizing any repairs beyond removal/replacement and the use of Concrete Jackets as allowed by Index 280.

Pipe Deficiency Summary Spreadsheet  

This new version provides columns which clarifies Contractor Proposed Remedial Action, CEI or Inspector Comments as well as what remedial action was performed and when it was performed.
