Access Request Instructions

 

Step 1: User Information

Please fill in the user’s name, phone number, cost center number (if applicable) and name, e-mail address and phone number. If you are a current employee or consultant, please fill-in your existing user id.

 

Step 2: User’s Acceptance of Conditions


Please read the following provisions:

Chapter 119, Florida Statutes, Public
Section 281.301, Florida Statutes – Safety and Security
Chapter 282, Florida Statutes – Communications and Data
Chapter 282.318, Florida Statutes – Security of Data and Information Technology Act
Chapter 815, Florida Statutes – Computer Related.

 

Also, read and complete the following:

• FDOT Computer Security Awareness Course.  The Flash player plug-in is required for this.

Step 3: User’s Signature

After completing Steps 1 & 2, please sign and date. The user’s signature is required to process this request.

 

Step 4: Request Type

Please check one that applies to you: new user, name change, access change, termination, or transfer.

If terminating or transferring, please fill-in the effective date, effective time, name, userid, and phone number.

 

Step 5: New Account Type

If a new user, please check the type of account requested: employee/OPS, consultant/contractor, generic userid, corporate, outside agency, FTP only, or other.


Step 6: Type of Computer Access Requested

Please check all that apply. If not sure what access is needed, please check with your immediate supervisor or manager.

If needed, approved access to buildings and access hours should be documented by the responsible manager. Please contact your office coordinator to request building access.

 

Step 7: Managerial Approval

(Corporate accounts are not required to fill this out.)

The requestor’s supervisor, cost center manager, and security coordinator’s signatures are required to process the FDOT Computer Security Access Request form. Please make sure the requestor, cost center manager, and security coordinator prints, signs and dates this section.

Step 8: For Consultant/Contractor Access Only

(This section is for consultants and contractors only.)

Consultants and Contractors please complete this section in its entirety. All information requested in this section is required.

 

Step 9: Routing Instructions

(Corporate accounts are not required to fill this out.)

Please check all that apply and enter all necessary information to promote effective routing.
